肿瘤专科医院胸外科住院日超过30天影响因素分析

• 【摘要】背景与目的 分析胸外科住院患者资料，明确住院日超过30天的主要原因。方法 收集2010年1月1日至2013年12月31日中国医学科学院肿瘤医院胸外科出院患者住院信息，采用构成比分析方法进行统计分析。结果 住院日超过30天主要集中于50～80岁（85.61%）、食管与食管胃交界肿瘤（73.23%）和肺部肿瘤的患者（25.00%）。住院日延长的首要影响因素为术后并发症其余因素依次为重要检查等待、术前合并症会诊等待、非手术工作日等待、医疗纠纷及转诊困难。结论 术后并发症是延长住院日的首要因素，食管及食管胃交界肿瘤患者术后并发症应受到重视，重要检查等待对住院日影响也较大，因此抓好围手术期医疗质量，提高医院各部门协同运行效率，对缩短平均住院日有重要意义。
• Background and ObjectiveInvestigating and analyzing influential factors about patients over 30 bed days in thoracic surgery department of Cancer Hospital of Chinese Academy of Medical Sciences. Methods：Relevant information of all patients in our hospital was collected from January 1, 2010 to December 31, 2013. Influential factors which extended the average length of stay were analyzed by Composition ratio analysis method Results Patients over 30 bed days concentrate mainly on those who suffers from esophageal neoplasms or esophagogastric junction tu mors (73.23%) and pulmonary tumors (25.00%) at the age of 50 to 80 years old (85.61%). Postoperative complication is t he primery influenctial factor which extends the average length of stay，followed by important check delay, the consultation of complex pr eoperative complications delay, the non-surgical day delay, medical dispute s and referral difficultis sequentially. Conclusion: Postoperative complication is the primery factor of patients ho spitalization extension. The postoperative complication of esophageal neoplasms or esophagogastric junction tumors should be seriously concerned. Important check delay has great influence on patients hospitalization. Paying special attention to medical quality，ensuring the medical quality and medical safety ，improving co-work efficiency of different department in hospital ha ve important meaning for hospital in shortening its average length of stay.
