白蛋白结合型紫杉醇单药治疗老年晚期非小细胞肺癌的临床观察

• 【摘要】目的 探讨白蛋白结合型紫杉醇单药治疗老年晚期非小细胞肺癌的耐受性和临床疗效。方法 回顾性分析2011年10月至2014年11月在中国医学科学院北京协和医学院肿瘤医院内科治疗50例老年晚期非小细胞肺癌（NSCLC），均接受白蛋白结合型紫杉醇化疗，具体方案：白蛋白结合型紫杉醇260毫克/m2，静脉点滴，第1天或分为第1、8两天；21天为一个治疗周期，至少完成2个周期后评估疗效。结果 50例患者均可评价疗效，客观缓解率（ORR）为22%，疾病控制率（DCR）78%，白蛋白紫杉醇的疗效与患者既往是否使用过紫杉类药物、病理类型及几线治疗等因素无相关性，P＞0.05。中位无进展生存时间（PFS）为4.0个月，中位总生存时间（OS）为12个月。本研究主要影响患者的不良反应为骨髓抑制、恶心呕吐、乏力、末梢神经毒性及肝肾功能损害。大多数不良反应为Ⅰ-Ⅱ度，耐受性良好。结论 白蛋白结合型紫杉醇单药治疗老年晚期非小细胞肺癌疗效肯定，耐受性良好。
• Objective:To discuss the Clinical curative effect observation and tolerance of nab-paclitaxel monotherapy in elderly advanced non-small cell lung cancer(NSCLC).Methods:From October 2011 to November 2011, 50 patients of elderly advanced NSCLC in Cancer Hospital Chinese Academy of Medical Sciences，who received chemotherapy of nab-paclitaxel monotherapy were retrospectively analyzed.All Patients were treated with 260mg/m2 nab-paclitax on day l, or divided into 1, 8 for two days.The cycle was repeated every 21 days.Clinical efficacy was evaluated every two cycles.Results:50 patients can evaluate curative effect. Objective response rate (ORR) was 22%, the disease control rates(DCR) 78%.The curative effect of nab-paclitaxel has no correlation with the pathologic types，the use of yew drugs before and the therapy on which line. The median progression-free survival(PFS) was 4.0 months and the median overall survival(OS) was 12 months.The major adverse reactions included myelosuppression, nausea, vomiting, fatigue, Peripheral neurotoxicity and kidney function damage. Most adverse reactions wasⅠ-Ⅱdegree.Conclusion:Nab-paclitaxel monotherapy in elderly advanced non-small cell lung cancer has good efficacy and tolerability.
