回盲部恶性肿瘤合并急性阑尾炎的临床分析

• 【摘要】目的 探讨回盲部恶性肿瘤合并急性阑尾炎的临床特点。方法 选取2010年2月至2016年1月在我院治疗的回盲部恶性肿瘤合并急性阑尾炎患者33例，分析患者临床资料，并随访患者情况。结果 33例患者临床症状中，转移性右下腹痛、右下腹压痛、恶性呕吐、腹肌紧张和反跳痛的比例分别为100.00%、81.82%、72.73%、72.73%和63.64%；病理检查显示，28例为腺癌（84.85%），5例为肉瘤（15.15%）；阑尾炎分型中，化脓性阑尾炎18例（54.55%），急性单纯性阑尾炎14例（42.42%），坏疽性阑尾炎1例（3.03%）；术后随访显示，1、3和5年存活率分别为89.29%、64.29%和53.57%。结论 应提高对急性阑尾炎及其并存病，特别是合并回盲部肿瘤的警惕性和认识。
• Objective: To investigate the clinical characteristics of the patients with ileocecal region malignancies complicated with acute appendicitis. Methods: From February 2010 to January 2016, selected 33 patients with m ileocecal region malignancies complicated with acute appendicitis in our hospital, analyzed the clinical data and followed up the patients. Results: The clinical symptoms in 33 cases, the percentage of metastatic right lower abdominal pain and right lower abdominal tenderness, nausea and vomiting, muscle tension and back pain were 100%, 81.82%, 72.73%, 72.73% and 63.64%; Pathological examination revealed 28 cases of adenocarcinoma (84.85%) and 5 cases of sarcoma (15.15%); 18 cases of suppurative appendicitis (54.55%), 14 cases of acute appendicitis (42.42%), 1 cases of gangrenous appendicitis (3.03%); Follow up showed that 1, 3 and 5 year survival rates were 89.29%, 64.29% and 53.57%, respectively. Conclusion: We should improve the vigilance and awareness of acute appendicitis and its concomitant disease, especially in the ileocecal region malignancies.
