乳腺肌纤维母细胞瘤的临床诊治分析

• 【摘要】目的：总结探讨乳腺肌纤维母细胞瘤的临床特点及诊治方法 方法：通过回顾性分析中国医学科学院肿瘤医院乳腺外科1999年1月至2013年3月之间于我院初治的共3例乳腺肌纤维母细胞瘤患者的临床病例资料并进行随访，分析总结本病的临床病理特点及诊治手段。 结果： 3例病人均为绝经前女性，平均年龄33岁，临床表现均为乳腺单发的无痛性肿块，大小均小于2cm，腋窝均无肿大淋巴结。影像学均表现为乳腺单发肿物，但并无特征性表现。治疗均选择手术切除，术后均未行其他辅助治疗。术后石蜡病理证实均为肌纤维母细胞瘤，主要免疫组化指标SMA、vimentin及desmin均为阳性表达，S-100为阴性表达。患者术后均恢复良好并随访至2014年10月，随访时间分别为82个月、39个月及19个月，随访结果均未见复发及远处转移。 结论：乳腺肌纤维母细胞瘤是一种较少见的间叶来源的乳腺肿瘤。其临床表现及影像学表现无特殊,确诊往往依靠术后石蜡病理及免疫组化指标(SMA、Vimentin和desmin、S-100等)。本病预后良好，治疗以手术切除为主，较少复发。
• Objective: To investigate the clinicopathological characteristics and treatment of breast myofibroblastoma(MFB).Methods: The clinical and pathological data of breast myofibroblastoma of 3 patients was reviewed in cancer institute& hospital , Chinese academy of medical science from 1999 to 2013,to analyze the clinicopathological characteristics and therapy of this rare tumor.Results: All 3 patients were premenopausal women with average age of 33 years old. MFB is presented by both palpation and imaging examintion as a solitary,unilateral,painless,freely movable nodule in all patients, without axillary swollen lymph nodes. All 3 patients were treated with tumor resection, and were confirmed by pathology as myofibroblastoma. In immunohisttochemistry, the expression of SMA, vimentin and desmin were positive, wherase the expression of S-100 was negtive. No postoperative adjuvant treatment were performed. All 3 patients were well recoveried and connected. By Oct. 2014, neither recurrence nor metastasis was observed in all 3 patients at the following-up time of 82 months, 39 months and 19 months, respectively.Conclusions: Myofibroblastoma of the breast is a rare benign stromal tumor with a good prognosis . Tumor resection is the applicable treatment . The diagnosis must be based on pathological characteristics.Application of immunohistochemical markers(SMA,Vimentin,desmin and S-100)may be useful in the diagnosis.
