前列腺癌骨转移的相关因素分析

• 【摘要】[摘要] 目的 分析前列腺癌患者的临床资料，探讨前列腺癌骨转移的相关因素。方法 以SPECT全身骨显像为诊断前列腺癌骨转移的金标准，回顾性分析2012年9月—2014年9月收治的235例前列腺癌患者的临床资料，采用单因素及多因素Logistic回归分析的方法分析前列腺癌患者年龄、Gleson评分、血清前列腺特异性抗原（TPSA）、血清游离前列腺特异性抗原（FPSA）及血清F/T值与骨转移的相关性 结果 单因素分析显示年龄、Gleason评分、TPSA、FPSA、F/T值及伴随淋巴结转移在前列腺骨转移组中差异具有统计学意义（P＜0.05），多因素Logistic回归分析显示，年龄(OR=1.08)、Gleason评分(OR=2.90)、TPSA值(OR=1.05)与前列腺癌骨转移具有明显相关性，其中年龄≥70岁、Gleason评分≥8分、TPSA＞20ng/ml和F/T≤0.16组中骨转移的患者明显多于其他组，差异均具有统计学意义（P＜0.05） 结论 年龄、Gleason评分及TPSA值是前列腺癌骨转移的主要预测因素。年龄≥70岁、TPSA＞20ng/ml、Gleason评分≥8、F/T≤0.16或者伴随淋巴结转移的前列腺癌患者应该行SPECT全身骨显像检查。
• 【Abstract】 Objective Analysis of the clinical data of patients with prostate cancer，To investigate the factors related to bone metastasis of prostate cancer. Methods On SPECT bone scan is the gold standard for the diagnosis of prostate carcinoma with bone metastas . Retrospective analysis of the clinical data of 235 cases of prostate cancer in 2012 to September 2014，by using multivariate Logistic regression analysis of 235 prostate cancer patients age、 Gleson score、 serum prostate specific antigen、serum free prostate specific antigen 、serum F/T level and Lymph node metastasiss correlation with bone metastasis . Results Univariate analysis showed that, in the prostate of bone metastasis and the difference was statistically significant age, Gleason score, TPSA, FPSA and F/T values（P＜0.05），Multivariate Logistic regression analysis showed that age(OR=1.08), Gleason score (OR=2.90）,TTPSA values (OR=1.05) are the independent risk factors and ≥70 years 、Gleason score ≥ 7 score、TPSA＞20ng/ml、F/T≤0.16 group of bone metastasis in patients with significantly more than the other groups,there were significant differences(P < 0.05)Conclusion Age、Gleason score and TPSA values were independent predictors of bone metastasis of prostate cancer , Gleason score and TPSA.≥70 years 、TPSA ＞ 20ng/ml 、 Gleason score ≥ 8 、F/T≤0.16 and Lymphonodus metastasis of patients should be for SPECT bone scan.
