子宫血管周上皮样细胞肿瘤的病理特征分析

• 【摘要】【摘要】 目的 探讨子宫上皮样血管周细胞肿瘤（PEComa）的形态学、免疫组织化学表现以明确诊断及避免误诊。方法 对8例子宫血管周上皮样细胞肿瘤进行病理学回顾性回顾性分析，并复习国内外文献。结果 瘤细胞大部分呈圆形和多角形上皮样，小部分呈梭形，胞质透明至淡嗜酸性，围绕血管生长，大部分病例HMB45和SMA强阳性。结论 子宫PEComa是一种罕见的恶性潜能未定的间叶性肿瘤，易误诊为平滑肌瘤，此外还需要与子宫内膜间质肉瘤、透明细胞癌及恶性黑色素瘤鉴别诊断，手术是最有效的治疗方法，对预后尚没有评估依据。
• [Abstract] Objective To investigate the morphological features, immunophenotypes of uterine perivascular epithelioid cell tumor (PEComa), thus to make a definitive diagnosis and avoid misdiagnosis. Methods Eight cases of uterine PEComas were retrospectively analyzed, and the related literature was reviewed. Results The tumor was mainly composed of round to polygonal cells with a small proportion of spindled cells. They have clear to slightly eosinophilic cytoplasms and display a perivsacular distribution. Most cases showed strongly positivity for HMB45 and SMA. Conclusion Uterine PEComa is a very rare mesenchymal tumor with uncertian malignant potential, which is easily misdiagnosed as leiomyoma. Differential diagnosis includes endometrial stromal sarcoma, clear cell carcinoma and malignant melanoma, as well as aforementioned leiomyoma. Surgery is the most effective treatment and more clinical experiences and data are needed for prognosis.
